  1. Your 7-digit meter number — exactly 7 digits as shown on your meter.
  2. Your full name and contact number.
  3. Your location, section/lot, and city/town.
  4. Whether your meter is Single Phase or Three Phase.

This form is only for 7-digit (CTS) meters. If you have an 11-digit meter and paid for a token, please use the main Claim Registration page instead.
